Renew Visa

Alhamdulillah, menulis lagi hari ini. Kali ini jugak nak kongsi tentang buat visa di mesir.  Selalunya visa student Malaysia di sini tahan selama setahun, jadi pada tahun hadapan memang kena renew semula. Tempat untuk renew visa ni di mugamma’, yang terletak di Tahrir, biasanya renew visa dibuat pada waktu pagi, supaya mudah dan tak perlu nak beratur panjang. Tambahan pulak mugamma’ ni tengahari  je dah tak berapa nak boleh renew kerana office hour diorang sampai tengahari ataupun sampai lepas zohor je. So nak x nak, memang kena gerak awal.

On the way nak ke mugamma’ ni, terpaksa menggunakan tren, orang mesir panggil metro. Lokasi tahrir dari rumah tidak jauh, hanya 2 stesen sahaja. Cumanya, keadaan di metro selalunya sarat dengan orang ramai, kalau orang sedikit, itupun pada waktu pagi ataupun tengahmalam.

Mugamma’ selalunya buka pada pukul 8 pagi, kalau sampai awal sebelum pukul 8 pagi, memang kena tunggu beratur diluar sampai pukul  8 pagi, kemudian baru dibenarkan masuk. Bila nak renew visa, kena isi borang  yang memang tersedia di atas kaunter. Even kita datang awal, ada juga orang lain yang datang awal, so, awal-awal lagi memang akan penuh mugamma’ ni, dalam setengah jam, akan penuh orang beratur. Ramai juga pelancong asing datang ke sini, atas urusan visa, dan kadang-kadang ada juga yang ada urusan kehilangan passport, dan macam-macam lagi.

Lepas isi borang, kena hantar dokumen-dokumen yang diperlukan, antaranya salinan fotostat passport, syahadah (macam surat approval untuk buat visa), salinan visa paling latest, sekeping gambar dan chop ketibaan sewaktu di airport mesir, kemudian kena bayar setem 8LE egyptian pound. Kemudian borang tersebut berserta dokumen akan diserahkan kepada pegawai untuk proses selanjutnya. Selepas itu, kena datang semula ke mugamma’ selepas 10 hari untuk mengambil semula, kemudian waktu tu, kena bayar lagi 83.10LE, after that kena tunggu selama 2 jam, then baru boleh ambil dan visa sudah selamat diperbaharui. Kalau lebih tarikh luput, memang kena denda, sebanyak 150LE.sebab pernah kena  J

Xpernah rasanya tetibe bersemangat nak buat post malam2 buta macam ni especially bila baru balik on call kat Emergency Department. Memang burnt out la sebab Emergency ni memang hectic especially at night.

But today, I’m not gonna talk about EM at all....but about something else which is quite interesting

So tadi bila log in email, I got one email from Elsevier. It’s a mail about one new app developed by them. The app is called Care of Muslims Patient- A Practical Guide.

So basically this app is about Fiqh Medic la lebih kurang about do and don’t that we as a healthcare people especially to know when we deal with muslims patient. Apart from that, this app also provides the medical profession with the information needed to appreciate the concerns of this community and thus improve the delivery of healthcare. 

App ni bukan calang2 olang yang buat tau? It is a JV program between Elsevier Singapore with Federation of Islamic Medical Associations (FIMA)

Alhamdulillah dapat lagi hidup pada hari ini untuk meneruskan hidup sebagai khalifah di bumi ini. Lama sudah tidak menulis, jadi rasa macam bermula dari bawah pun ada. Semoga diberi kekuatan untuk istiqamah dalam jalan ini.

Sedar tak sedar, dah hampir 5 tahun di bumi mesir, dan kali ini juga masuk kali kelima musim sejuk di sini. Rasa macam baru semalam je smpai mesir, dengan muka-muka fresh year tahun 1, tapi sekarang sudah berada di tahun 5. Cepat sungguh masa berlalu.

Mesir ni negara 4 musim, jadi tak terlepas dari 4 musim, spring, winter, fall dan summer, tapi musim utama di sini adalah spring, winter dan juga summer. Fall di sini tak macam di negara lain, tempohnya sekejap, dan ciri-ciri daun luruh itu memang ada. Winter di sini juga lain, sebabnya, takde salji, dan kesejukan adalah yg utama bagi winter di sini. Tahun ni winter bermula awal November, dengan hujan. Hujan di sini mostly sekejap sangat2, bila hujan, itu petanda mesir mengalami perubahan musim. Perubahan suhu di sini berlaku secara gradual, dari sejuk yg boleh ditahan, kepada sejuk yang tak tertahan.

Kalau winter di sini selalunya memang akan pakai sweater selalu, kalau di rumah pula, akan pasang pemanas bilik atau nama lainnya heater, dan sakhonah (pemanas air). Dan seperti biasa, akan dihinggapi selsema dan demam dek kerana perubahan suhu, dan selalunya juga akan kurang mandi, kerana mandi kerap sewaktu winter adalah sesuatu yang tidak baik untuk kesihatan, dan juga disarankan untuk memakan buah limau, kerana kaya dengan vitamin c yang mana sumber perlindungan waktu winter ini dari penyakit. Time winter jugak waktu malam menjadi lebih lama dan waktu siang lebih pendek, jadi kepada orang-orang yang puasa, ini merupakan satu kelebihan yang Allah bagi kepada mereka untuk istiqamah dengan puasa sunat.

Hallucination

Post hari ni berkaitan dengan Psychiatry sikit.

Seorang junior pernah mengajukan soalan kepada sy bertanyakan tentang perbezaan diantara hallucination dan delusion.

Ada beza ke???

Ye…memang ade beza (^_____^)

So what is hallucination?

Hallucination ni merupakan persepsi yg tebentuk dalam minda kita tanpa wujud sebarang stimulus, dan persepsi tu terjadi dalam keadaan kita sedar. Persepsi tadi juga mempunyai kualiti yang hebat, seolah2 apa yg kita halusinasikan tu betol2 terjadi dan wujud di hadapan mata kita!



Hallucination ni banyak jenis pulak. Kat bawah ni sy sediakan beberapa contoh2 untuk memberi lebih kefahaman kepada sahabat2 semua.


  •  Auditory hallucination ( halusinasi bunyi): Anda sedang berada dalam sebuah bilik tertutup yg sunyi. Hanya anda seorang sahaja yg berada di situ… tiba2 anda terdengar ada suara berbisik di telinga anda…ataupon anda terdengar foot steps…hehehe. Auditory hallucination is the MOST COMMON TYPE OF HALLUCINATION.
  • Gustatory hallucination ( halusinasi rasa): Dalam mulut anda takde sebarang makanan, tetapi lidah anda dapat merasa sesuatu secara tiba2…mcm ayam goreng ke….
  • Olfactory hallucination ( halusinasi bau): Hidung anda tiba2 tercium bau (selalunya bau busuk but pleasant smell pon boleh jadi jugak) padahal di sekeliling anda takde pon ape2 yg boleh emit bau busuk sebegitu
  •  Somatic/tactile hallucination ( halusinasi sentuhan): Anda terasa bagai di sentuh, sedangkan takde sesape pon yg sentuh atau tersentuh anda. Kalau tactile ni plak, rasa mcm ade bug crawling kat bawah kulit anda. Tactile hallucination ni common among intravenous drug user (IVDU) a.k.a penagih dadah. Most of the time drugs yg hallucinogen adalah heroin and morphine.
  •  Visual hallucination ( halusinasi penglihatan): Melihat kelibat seseorang yg tidak wujud pon di tempat kejadian


Hallucination is one of characteristic symptoms to diagnose Schizophrenia; ONE OF THE CHARACTERISTIC means x semestinya anda ade one of the above dah boleh di kira sebagai Schizophrenia ok? (^__^)

Agaknya ape penyebab utama berlakunya hallucination?

Hallucination terjadi mostly kepada mereka2 yang mempunyai masalah mental seperti Schizophrenia, Bipolar Mood Disorder (BPMD), major depression (kemurungan) or even dementia (nyanyuk). Di samping itu, pengambilan dadah, stress dan brain disease pon boleh membawa kepada hallucination sebab kesemua keadaan di atas akan menjejaskan fungsi otak yg normal which eventually may lead to hallucination.
Boleh dikatakan ramai daripada kita tak tahu mengenai Charles Lutwidge Dodgson a.k.a Lewis Caroll, tapi kalu sebut je Alice in Wonderland mesti semua orang tahu kan?

Hallucination is so synonym with Lewis Caroll. Do you know why?

Lewis Caroll ni merupakan penulis asal buku Alice in Wonderland. Some scientist suspect and claimed that Lewis Caroll suffers from liliputian hallucination. If a person is having liliputian hallucination, they will have wrong perception about size. Mereka akan nampak sesuatu objek itu boleh mengecil dan membesar.



The scientist suspect it to happen when they saw how Lewis Caroll portray the story of Alice in Wonderland dimana Alice herself boleh mengecil dan membesar. Lepas je buku ni di published scientist menamakan syndrome tersebut sebagai Alice in Wonderland syndrome.

Cukup la dulu kot pasal hallucination.

Memang tahun ni x berapa nk produktif la blog ni sebab berapa sen je post yang tersiar. Jutaan ampun dan kemaafan sbb kami x sempat nk update blog. Alhamdulillah hari ni ade sedikit masa dan idea ntuk menulis so terhasillah post ni….

ORTHOPEDIC!!!



Hari ni saya nk cerita sket pasal ortho sebab now I'm in Ortho posting (promote2) ^^,

So basically Orthopedic is one of the fields in medicine which involve management as well as prevention of problems related to bones, muscle, joint and ligament; from dislocation to fracture, sports injury, congenital malformation till bone tumor. Memang vast sangat bidang pengajian Orthopedic ni. 

Ape yang kita study dalam ortho?
Problems dalam ortho ni bukan sahaja melibatkan Motor Vehicle Accident(MVA) or kemalangan semata2. Kadang kala, ortho jugak merawat pesakit2 yang lahir dengan disability related to tulang seperti club foot or rickets. Diabetic patient yang ade diabetic foot ulcer (DFU) and required amputation pon dirawat di Orthopedic department jugak kadang2… so memang various cases la from trauma, medical and surgical base.

Best tak ortho?


Kan dah kate...ade math and physics?

Best sebab Ortho ni banyak berkait dengan Math dan physics. Nak kate berkait sangat tu takde la tapi dalam Ortho kita akan study banyak angle, dan kita kena paham konsep pulley and load betol2 sebab management2 patient dalam Ortho case banyak menggunakan konsep physic. Kalau masuk je ward ortho, kita boleh tengok banyak pulley yang tergantung di sana sini.

Dalam OT pulak…. Anda akan merasakan diri anda seperti berada dalam bilik kemahiran hidup bersepadu (KHB). Memang banyak besi, spanar, tukul dan berbagai2 lagi gerudi dan accessory2 lain.  Memang seram la jugak nampak depa gerudi kaki orang….especially bagi case2 yang perlu masukkan besi ( medically kita x panggil besi… tapi internal fixator) tapi lama kelamaan akan jadik biasa ja.




Macam mane nak buat specialist dalam bidang ni?

Ok…actually lepas je korang abis MBBS, houseman dan dah bergelar MO, boleh la mintak sambung master dalam bidang Orthopedic ni. Lepas dah settle buat master, so korang pon bergelar specialist la. Bunyi cam senang tapi susah wooo….

So itu la serba sedikit pasal Orthopedic yg sy sempat kongsikan hari ni. Tomorrow is my last day in Orthopedic ward kat HTAR Klang and insyaAllah hari Jumaat ni berakhirlah my Orthopedic posting untuk tahun 4.
Jumpa agi ye ortho next year….insyaAllah (^________^)

P/s: Perkataan paling menarik dalam Ortho is Ilizarov... buleh bagi nama anak. Nak tahu x ape bende Ilizarov tu sila tengok kat bawah ni...hehe. Device ni la namanya Ilizarov. Chantek kot nama...tapi procedurenya SERAM.


Stop Child Abuse!!!

Assalamualaikum w.b.t and salam sejahtera..

Alhamdulillah today, saya baru je balik dari Seminar One Stop Crisis Centre anjuran OSCC Unit & SCAN team of HSB. This program is basically about how we as medical doctors cater child abuse cases. 

Regardless of the types of abuse, this seminar cover all aspect by giving some brief guideline how to manage patient that came to us with the chief complaint of abuse.Memang best!!!



Many of us regard a word ‘abuse’ as a taboo in our society. But as the time flies, the abuse cases especially related to child abuse are increasing day by day. It’s sad to know that people are reluctant to discuss this matter and they keep on closing their mind. So let’s have some anjakan paradigma starting from today by opening our mind and and have some knowledge what abuse is all about, ok? 

So…

Abuse is defined as any sort of assault; maybe physical, sexual, emotional, negligence, exploitation or trafficking that might alter the health, life as well as the child’s dignity. Abuse is not only sexual ok! For example:

  • A mother who neglect the daily needs of her child; for example clothing, feeding, schooling is considered as abuse.
  • Pemerdagangan orang (like jual anak...jual isteri…) also considered abuse.
  • Menggunakan kanak- kanak untuk meminta sedekah di kira sebagai exploitasi which is also considered as abusive.
  • Rakan sekerja or boss kat pejabat yg suke usha lain macam and salu sentuh2 anda…or buat isyarat yg xsepatutnya ( u know what I’m saying…) di tempat kerja is consider sexual abuse.
So instead of understanding abuse as only sexual abuse, there are actually a lot of other types of abuse that we might encounter daily but we missed it.

So as for the seminar per se, there were 9 speakers which talks about physical and sexual abuse in children. The speakers came from various department; Pediatric, Obstetric & Gynecology, Forensic Medicine, Psychiatry. This seminar covers all aspect of management of abuse cases patients. 

There were even invited speakers from PDRM; DSP Choo Lily…memang best sangat dia punye talk. Dah macam Inspektor Aleeza dalam Gerak Khas pon ade. Theee (^^,)… Apart from that, we also have sessions with a speaker from JKM, a Psychologist from JKNS and the representative from All Women’s Action Society (AWAM).

We gain a lot of new information from the speakers today... thanks to Pediatric Specialist, Dr. Zahilah Filzah for the invitation.

Banyak sebenarnya hal berkaitan child abuse yang kita tak tahu, so dalam entry2 lepas ni I try my best to share some important examples of child abuse…insyaAllah (^___^).

So to put in a nutshell, child abuse is not a personal problem. It is a nationwide problem that needs all available hands to stop it spread further in the society. Say no to CHILD ABUSE!!!

Medical Textbook

Salam alaik and salam sejahtera…
I personally wanna congratulate all 1st year juniors as they already started their MBBS course this coming Monday. To other batches especially 3rd year, welcome aboard to clinical life where real things happen.

So basically this post is about question asked by my fellow juniors regarding textbooks. I hope this post may benefit us altogether. 

The standard international book will always be the same where ever you go. However there are some special preferences from each lecturer based on their own experience which surely will help us. As we are having a lot of lecturers abroad, so there are some books recommended by them and I personally find it so easy to use and easy to understand all the facts. Some notes regarding books:
  • 1st year
o   Anatomy they will have 4 types of books; Gray’s Anatomy textbook, Netter atlas (atlas is some sort of kamus bergambar), histology book as well as embryology book.
o   Physiology will use the Guyton.
o   Biochemistry using books by U Satranarayana but some other international book also can be use. 

  • 2nd year
o   Pathology usually we use Robbins and Cotran. We also had textbook from India. The author is Harsh Mohan. For Pathology u might need to have atlas as well.
o   Microbiology book is by Jawetz but I personally used Microbiology textbook by Chakraborty.
o   Forensic we can use Krishnan Vij. There is also another book written by Gautam Biswas which is a review of Forensic Medicine.
o   Pharmacology suggested book is Lippincott, Katzung as well as K.D Tripati.



  • 3rd year (during my time)
o   Medical we can use Kumar & Clark or Davidson.Some handbook like oxford handbook also will be easy to carry around and so helpful especially when we attend clinic session.
o   Surgery is either Browse or the best one is Bailey & Love.
o   For O&G we can use Obstetric and Gynecology by Ten Teachers.
o    For pediatrics, there are two books; Nelson and Illustrative Pediatric or the popular name is Sunflower book.We also must have a pediatric protocol by MOH.
o   PCM can use John Murtagh.
o   Psychiatry can use DSM IV book, Kaplan & Saddock as well as Psychiatry clerkship book.


 I hope that it may help some of you to choose which book to read. Beli dan bace ye? Jangan jadik mcm saya...having a stack of unread book at home. Haha..

Selamat Tinggal Tahun 3

It’s been a while since I update this beloved blog of ours.

Semoga semua pembaca berada dalam keadaan sihat walafiat serta senentiasa berada dlm ridha Allah. InsyaAllah…

Esok officially sy akan masuk tahun 4. The second year of clinical years..Alhamdulillah…

Kesemua suka duka di tahun 3 sudah melabuhkan tirainya. Keputusan exam pon dh keluar. Syukur Alhamdulillah, tahun ni result better than tahun lepas. Alhamdulillah lulus ke enam2 matapelajaran.
Sekadar menggamit kembali kenangan di tahun 3…

I had been posted to Hospital Klang during my 3rd year. My first posting was Obstetric & Gynecology. Masa mula2 nak masuk tahun 3, saya berdoa supaya Surgery menjadi 1st posting saya and O&G jadik yang last. But cliché plak rasa other way around yg happen.. so not happy at that moment.
But bila dh masuk O&G, I fell in love with the subject. And I cried twice masa tengok baby di lahirkan. Feel so gifted at that moment sebab I live as a women. (^__^) Subhanallah…

My 2nd posting, which I must say the toughest among all other subjects was Pediatric. I had a lot of bad experience with kids and I suffer during the posting. And I suffer until exam. I got a very cooperative patient but a super doper scary examiner T__T… but yet Alhamdulillah I managed to pass. Sebbek...hehe.

3rd posting is Primary Care Medicine (PCM) posting and Psychiatry. Both are 1 month duration punye posting. I don’t remember much about PCM. Apa yang sy bleh recall only during the exams where u should make a diagnosis and consult the patients. It’s so scary and I feel so terrible because the patients on the day of the exams were actually medical doctors as well. So kalau U goreng cakap dia ade hyperlipidemia but the truth is he doesn’t, memang jadi bahan la…LOLs.

Medicine followed later on for about 2 months. I had a lot of hardship because posting2 lain, report yang kena submit was only 2. But for this posting, it is 42 hand-written case reports. Memang malam deadline tu, me and my hosmate semua stay up…sampai lenguh tangan menulis 42 case. Btw, sape suh bertangguh kan??? T__T …But thanks to the lecturers who forced us to do it, we learn a lot in Medicine and I miss it so much (^^,).

My favorite subject, SURGERY...SURGERY…SURGERY!!!

During my postings, we are so lucky to have visiting lecturers from M.S Ramaiah, Bangalore. Surgery ni memang best kalau kita minat sbb it’s a matter of skills and techniques. We study how to diagnosed a surgical cases like hernia and we also had some ideas about the management of each cases. U wanna noe more? Maybe u can click this LINK and read about it okes??
Habis sudah tahun 3…

Syukur pada Allah yang telah memberikan saya kemudahan dalam kehidupan saya selama ini…
Thanks to my parents, my family members, my teachers and all my fellow comrades of 3rs year MBBS IMS Shah Alam. I feel so honored to be a part of it and to have a chance to sail with u guys.

So long 3rd year…



And tomorrow…?
Tomorrow is a brand new day of 4th year… nak rajin2..xmo malas2 and x mo gak banyak tido dalam kelas (^__^)
